Short answer

In the technical rounds
Rambling instead of leading with a one-line answer; not clarifying requirements before coding; ignoring edge cases (loading/empty/error states); mutating state in React; using array index as a key on dynamic lists; and over-engineering a simple problem. Structure your answer, then go deep.
In the behavioural round
Vague stories with no result, saying 'we' instead of 'I', trashing a past employer, and having no genuine 'why this company' answer. Use STAR, quantify outcomes, and stay positive.
